N-acetylcysteine: NAC, glutathione, antidote use, COPD findings, conflicting OCD trials, GlyNAC, and supplement policy

The name and outcome distinctions
| Term | Meaning |
|---|---|
| NAC / acetylcysteine | Names for the compound in this article.[1] |
| Glutathione | An intracellular antioxidant studied in the GlyNAC work.[7] |
| GlyNAC | Glycine plus NAC; a combination exposure.[7] |
| Exacerbation | A COPD worsening episode; different from a lung-function measurement.[3] |
| Adjunct / augmentation | Added to another treatment; different from testing NAC alone.[6] |
A medical antidote indication is specific
The July 2025 ACETADOTE label describes intravenous acetylcysteine to prevent or lessen liver injury after potentially toxic acetaminophen exposure, including specified acute and repeated supratherapeutic ingestion settings. Its proposed mechanism includes maintaining or restoring glutathione and handling a reactive acetaminophen metabolite. That indication does not establish routine “detoxification” of an otherwise healthy person, or make an oral supplement a substitute for urgent poisoning care.[1]
BRONCUS did not improve its primary COPD outcomes
The 2005 BRONCUS trial randomized 523 people with COPD at fifty centers and followed them for three years. Its primary outcomes were lung-function decline and annual exacerbations. NAC did not significantly improve either compared with placebo. A subgroup without inhaled corticosteroids and a secondary hyperinflation analysis were more favorable, but neither overturns the primary result.[3]
The PANTHEON evidence has a different setting
An original 2019 post hoc analysis describes the earlier PANTHEON dataset as showing fewer exacerbations with a higher NAC exposure: 1.16 versus 1.49 episodes per patient-year. This contrasts with BRONCUS, but different populations, exposure, duration and accompanying medicines complicate direct comparison. A rate per patient-year is different from the percentage of people helped.[4]
The conflicting findings require comparison
The selected trials do not establish a single reliable effect across all OCD populations or other psychiatric diagnoses. Full eligibility, missing-data, multiplicity, sponsor roles and clinical-significance appraisal remain incomplete. A favorable interaction on a symptom scale is different from evidence of lasting remission, independent everyday function or superiority to established psychological care.[5][6]
GlyNAC findings cannot isolate NAC’s effect
A trial published in a 2023 issue randomized 24 older adults to glycine plus NAC or an alanine comparison for sixteen weeks. It reported biochemical and physical-function improvements. It did not separate the two ingredients or test survival, and twelve participants per older-adult arm cannot establish universal safety or reversal of human aging.[7]
The dated FDA policy is enforcement discretion
FDA’s final August 2022 guidance describes discretion for certain NAC-containing products labeled as supplements, provided other applicable requirements are met. It does not say every NAC product is an approved supplement drug, or approve disease-treatment claims. The guidance itself explains that it is nonbinding. This page identifies the policy and its date; a full subsequent legal-history comparison remains outstanding.[2]
Route-specific harms need route-specific interpretation
The intravenous product label warns about serious hypersensitivity and fluid overload. Those warnings cannot be converted into an oral-supplement adverse-event rate. Small oral trials also cannot establish indefinite safety, medicine compatibility or safety in every special population. Personal poisoning management, psychiatric care and NAC regimens are outside this explanation.[1]
Evidence & uncertainty
NAC is discussed as both a medicine and a supplement ingredient. FDA’s enforcement-discretion guidance addresses specified marketing circumstances, rather than proof that NAC treats every condition claimed online. A permitted or tolerated sale is a separate question from efficacy, product quality and individual safety.[2]

A later subgroup report is not another randomized trial
The PANTHEON reanalysis explores smoking status and concomitant medication. It uses the same underlying trial rather than a new replication. Selected original indexed result and design passages were available; complete subgroup prespecification, interaction testing and primary-trial appraisal remain pending. The BRONCUS record also lists an erratum whose full correction was not inspected; no disputed numerical lung-function interval is reproduced here.[4][3]
The 2022 OCD trial was negative
A multisite Australian phase III trial studied adjunctive NAC in 98 adults with OCD for twenty weeks. Its modified intention-to-treat primary analysis found no improvement over placebo on the Yale–Brown symptom scale. The mean difference was 0.53, with a compatibility interval from −2.18 to 3.23. This did not demonstrate benefit, while its uncertainty also prevents treating the groups as proven equivalent.[5]
A small 2026 trial was more favorable
A twelve-week pilot randomized 35 adults with moderate-to-severe OCD to sertraline augmented with NAC or placebo. It reported a favorable time-by-group interaction for overall symptoms and an early-response signal. It tested addition to sertraline, rather than NAC replacing treatment. Its smaller size and different duration and background care mean it cannot by itself settle the larger negative trial.[6]
